To receive a certificate of compliance for any product manufactured prior to and including May of 2011 please email compliance@foundations.com or call 330-722-5033 ext. 101.
To generate the certificate of compliance for your product manufactured June 2011 or later please enter the data in the Certificate Request Form. This information can be found on a label located on your product. For example, a wood crib product identification label is located on the headboard or footboard:
