To generate the certificate of compliance for your product manufactured June 2011 or later please enter the data in the form below. This information can be found on a label located on your product. For example, a wood crib product identification label is located on the headboard or footboard.

To receive a certificate of compliance for any product manufactured prior to and including May of 2011 please: email compliance@foundations.com or call 330-722-5033 ext. 101.
